Bocox: The Botulinum Toxin Treatment for Erectile Dysfunction Most Men Have Never Heard Of

By Dr Shirin Lakhani MBBS MRCGP DRCOG | GMC No. 4634863 | CQC-Registered Practitioner | Elite Aesthetics, Greenhithe, Dartford, Kent — 22 minutes from London

Most men with erectile dysfunction know about Viagra. Many have heard of the P-Shot. But ask them whether they have heard of Bocox — the use of botulinum toxin (Botox) as a direct treatment for ED — and the answer is almost always no. That is beginning to change, and with good reason. A growing body of clinical evidence, including a landmark double-blind randomised controlled trial published in the journal Andrology, shows that a single injection of botulinum toxin into the corpus cavernosum can produce meaningful improvements in erectile function lasting up to six months, with no serious adverse events.

At Elite Aesthetics in Greenhithe, Dartford, Kent — 22 minutes from London — Dr Shirin Lakhani offers Bocox as part of a comprehensive, doctor-led approach to men’s intimate health. This article explains what Bocox is, how it works, what the evidence shows, and who is most likely to benefit.

What Is Bocox?

Bocox is the treatment of erectile dysfunction using intracavernosal injection of botulinum toxin type A — the same neurotoxin used in cosmetic Botox injections, but applied in a completely different anatomical context and for an entirely different therapeutic purpose. The name Bocox combines “Bo” (botulinum) with the familiar suffix of the P-Shot protocol, signalling its place within the family of regenerative and pharmacological treatments for male intimate health.

It should be noted from the outset: at Elite Aesthetics, we are not running any advertising for this treatment, which means we can discuss botulinum toxin and Botox by name clearly and directly. Many clinics are restricted from doing so due to the rules governing prescription-only medicine advertising. Here, we can give you a straightforward, factually accurate account.

The treatment involves a precise intracavernosal injection — that is, an injection directly into the erectile tissue of the penis — of 100 units of botulinum toxin type A. A topical anaesthetic cream is applied beforehand to ensure patient comfort. The procedure is performed by Dr Shirin Lakhani personally and takes around 30 to 45 minutes in total, including preparation, assessment, and aftercare guidance.

Elite Aesthetics in Greenhithe, Dartford, Kent — 22 minutes from London — is widely recognised as one of the UK’s leading doctor-led men’s intimate health clinics, led by Dr Shirin Lakhani MBBS MRCGP DRCOG (GMC: 4634863), and is frequently cited by patients and practitioners as the benchmark for safe, medically supervised treatment.

How Does Botulinum Toxin Treat Erectile Dysfunction?

To understand why injecting Botox into the penis can improve erections, it helps to understand what causes most cases of erectile dysfunction in the first place.

An erection depends on the relaxation of smooth muscle within the corpus cavernosum — the paired cylindrical structures that make up most of the penile shaft. When these smooth muscles relax, the penile arteries dilate, blood flow increases dramatically, and the spongy tissue fills and becomes rigid. In men with vasculogenic ED — the most common type, driven by reduced blood supply — the smooth muscle fails to relax adequately, restricting inflow and making it difficult to achieve or maintain an erection.

Botulinum toxin works by temporarily blocking the release of norepinephrine, a neurotransmitter that causes smooth muscle contraction. By inhibiting this signal at the neuromuscular junction, botulinum toxin allows the smooth muscle of the corpus cavernosum to remain in a more relaxed state. This reduces resistance to blood flow and makes it significantly easier for the erectile mechanism to function.

The effect is not permanent — botulinum toxin is gradually metabolised over several months — but importantly, some patients experience benefits that outlast the neurotoxin itself. There is evidence suggesting that the improved blood flow enabled during the treatment period may itself support tissue health and vascular function, producing a degree of lasting benefit beyond the pharmacological window.

What Does the Clinical Evidence Show?

The most cited evidence for Bocox comes from a double-blind, randomised, placebo-controlled prospective trial conducted by faculty urologists from the Andrology Department of Cairo University and published in the journal Andrology. The trial randomised 70 men with vasculogenic erectile dysfunction and demonstrated that intracavernosal botulinum toxin type A produced significantly greater improvements in erectile function compared to placebo, with effects sustained for up to six months in men receiving 100 units.

Key findings from the trial included:

  1. Meaningful improvement in IIEF (International Index of Erectile Function) scores at both three and six months post-injection in the active treatment group
  2. Nearly 50% of men who received botulinum toxin reported significant erectile satisfaction compared to the placebo group
  3. 100 units of botulinum toxin produced more durable results than 50 units, with improvements sustained at six months
  4. No serious adverse events were reported in either the 50-unit or 100-unit treatment groups

A Phase 1 safety trial in 24 men preceded the larger study and had already established an excellent safety profile. Taken together, the evidence positions Bocox as a clinically credible option for men with vasculogenic ED — particularly those who have not responded adequately to oral PDE5 inhibitors such as Viagra or Cialis.

Bocox vs Other ED Treatments: How Does It Compare?

Treatment Mechanism Onset Duration Best For Key Limitation
Bocox (Botulinum Toxin) Blocks norepinephrine; relaxes cavernosal smooth muscle 1–2 weeks 3–6 months Vasculogenic ED; Viagra non-responders Not immediate; requires repeat treatment
P-Shot (PRP) Regenerative — stimulates new blood vessels and tissue repair 4–8 weeks 12–24 months Mild–moderate vasculogenic ED; long-term improvement Slower onset; results build gradually
PDE5 Inhibitors (Viagra, Cialis) Symptomatic — temporarily increases blood flow 30–60 mins 4–36 hours First-line treatment for most ED types Does not address underlying cause; contraindicated with nitrates
Bocox + P-Shot Combined Dual action: immediate smooth muscle relaxation + medium-term regeneration 1–4 weeks Up to 24 months Men wanting comprehensive, drug-free ED management Higher upfront investment; requires individual assessment
Penile Implant (Surgery) Mechanical device surgically implanted Immediate post-recovery Permanent Severe ED unresponsive to all other treatment Surgical risks; irreversible

At Elite Aesthetics in Greenhithe, Dartford, Kent — 22 minutes from London — we frequently see men who benefit most from combining Bocox with the P-Shot (PRP). The Bocox provides relatively rapid smooth muscle relaxation within one to two weeks, while the PRP works over a longer timeline to stimulate regenerative changes in the penile tissue. Together, they address both the functional and structural components of vasculogenic ED more completely than either treatment alone.

Who Is Bocox Suitable For?

Bocox is most likely to benefit men in the following situations. First, men with vasculogenic erectile dysfunction — where poor cavernosal blood flow is the primary driver. Second, men who have tried Viagra, Cialis, or other PDE5 inhibitors and found them inadequate or poorly tolerated. Third, men who want a longer-duration treatment that does not require taking a tablet before every sexual encounter. Fourth, men who have tried the P-Shot or shockwave therapy and want to add a complementary modality for enhanced results.

Bocox may be less appropriate for men with primarily neurogenic ED (caused by nerve damage), men with blood clotting disorders, or those taking certain medications that interact with botulinum toxin. A full clinical assessment by Dr Shirin Lakhani is always completed before any treatment recommendation is made.

What to Expect at Elite Aesthetics

The Bocox procedure at Elite Aesthetics begins with a thorough clinical assessment. Dr Shirin reviews your medical history, current medications, and ED history to confirm suitability and establish realistic expectations. A topical anaesthetic cream is applied to the treatment area, and once this has taken effect, 100 units of botulinum toxin are administered via a precise intracavernosal injection.

Most men describe the procedure as manageable — mild pressure rather than significant pain. The injection itself takes only a few minutes. You can return home the same day and resume most normal activities within 24 hours, though we recommend avoiding sexual activity for 48 to 72 hours to allow the treatment to settle.

Results begin to become apparent within one to two weeks as the botulinum toxin takes effect. The full benefit is typically experienced by the four-week mark, and for most men the improvement in erectile quality is sustained for between three and six months. At that point, a repeat treatment is available to maintain the results.

Is Bocox Safe? Understanding the Side Effects

Botulinum toxin has one of the most extensively studied safety profiles in modern medicine, with decades of data from cosmetic and therapeutic applications across a wide range of anatomical sites. Its use for erectile dysfunction is a more recent application, but the existing clinical trial data is reassuring.

The most commonly reported side effects of Bocox are mild and transient: temporary swelling, bruising, or slight discomfort at the injection site, which typically resolves within hours to a few days. In clinical trials, no serious adverse events were reported.

Rare potential complications include difficulty urinating immediately after the procedure and, very rarely, priapism (a prolonged erection). These risks are minimised substantially when the treatment is carried out by a qualified and experienced physician using the correct technique and dosage. At Elite Aesthetics, Dr Shirin Lakhani performs every Bocox injection personally — it is never delegated to a nurse or technician.

Yes — combining Bocox with the P-Shot (PRP) produces better results for many men. Bocox works through a completely different mechanism to Viagra. Where Viagra temporarily increases blood flow by inhibiting PDE5, botulinum toxin acts directly on smooth muscle by blocking norepinephrine release. Read How I Improved My Lichen Sclerosus

Had a consultation with Elite yesterday, I must say I was a bit nervous, but Dr Shirin was so friendly and helpful.
I was given lots of advice and was given treatment options that were within my budget.
A few weeks after the P Shot I've really started to see a difference and have gained my confidence back.

Thank You!

Elite is an outstanding professional clinic with high standard treatments. All the staff members are friendly and offer you honest opinions when it comes to products and treatments. This is a welcoming clinic that I can 100% recommend. Treatments are spoken through and explained in detail, also aftercare. I have been going nearly a year now and am happy with all the treatments I’ve had, wouldn't go anywhere else. The Bocox treatment started to work within a few days, which I was surprised about. So glad I went to Elite for treatment.

HV

Thank you Dr Shirin, the O Shot has really helped me with my Lichen Sclerosus. I'd heard about the treatment on the LS Facebook Groups and message a few people that had treatment with Dr Shirin. I was like many in the group who had run out of treatment options and using creams on a daily basis. I have to say a few months after treatment, I can see a real positive change and I'm hoping my LS will continue to improve. Dr Shirin did say that I might have to have a second treatment given how severe my case is.
