Lichen Sclerosus is a long-term skin condition that causes thin, white, itchy patches to develop on the skin. It most commonly affects the genital and anal areas, particularly the vulva in women.

There are many misconceptions about LS, and it is really important to understand that Lichen Sclerosus:

  • Is not contagious: You will not catch it from someone else or pass it on to anyone.
  • Is not caused by poor hygiene: LS is not related to cleanliness.
  • Is not a form of cancer: Although it can sometimes increase the risk of certain cancers if left untreated.

What Causes Lichen Sclerosus?

The exact cause of Lichen Sclerosus is unknown, which is partly why it is so difficult to treat. However, with this in mind, most doctors agree that several contributing factors could be responsible for its development, and they are listed below:

Hormonal changes: It’s more common in women after menopause, suggesting that lower oestrogen levels may play a role.

Overactive immune system: The body’s immune system may mistakenly attack healthy skin cells, leading to inflammation and skin changes.

Genetic factors: Doctors believe that there may be a genetic predisposition to developing Lichen Sclerosus, as it can sometimes run in families.

Previous skin damage: Injury or irritation to the skin in the affected area may trigger Lichen Sclerosus in some cases.

Symptoms of Lichen Sclerosus on Vulva, Anus & Labia

Itching: This is often the most bothersome symptom and can be severe and persistent.

Skin changes: The skin may appear thin, white and crinkled with possible red or purple patches.

Pain or discomfort: This can occur during sex, urination, or even just during everyday activities.

Bleeding or tearing: The skin becomes fragile and easily damaged.

Sexual difficulties: Pain and tightness can make sex difficult or impossible.

How is Lichen Sclerosus Diagnosed?

Lichen Sclerosus (on vulva, anus and labia) are notoriously misdiagnosed. Symptoms can often be confused with other conditions, and finding the correct diagnosis and treatment is difficult. However, if you suspect you might suffer from this condition, ask your doctor for an examination. An experienced gynaecological doctor will examine the affected areas and ask about your symptoms.

A small skin sample may be taken to confirm the diagnosis and rule out other conditions. As mentioned earlier, this condition can be mistaken for various other issues, which makes it difficult for many women to receive a proper diagnosis. Treatment Options for Lichen Sclerosus

Topical Corticosteroids: The most common treatment is topical corticosteroids, powerful anti-inflammatory creams or ointments that help reduce itching, redness and skin thinning. The most commonly prescribed are clobetasol propionate and mometasone furoate. 

However, it is essential to note that long-term use can have side effects, such as skin thinning, so they should be used only administered as directed by a doctor. These treatments can also become less effective over time as the body builds up a “tolerance” to them. 

Other Topical Medications:

Calcineurin inhibitors: These medications, such as tacrolimus and pimecrolimus, suppress the immune response and can be helpful for people who cannot tolerate or have not responded well to corticosteroids.

The O-Shot: The O-Shot, or Orgasm Shot, is a procedure that involves injecting platelet-rich plasma (PRP) into the clitoris and other areas of the vulva.
