HIFU vs Thread Lift for Facial Lifting: Which Is Better?
Two of the most sought-after non-surgical facial lifting treatments available today are HIFU and the thread lift — and for good reason. Both can turn back the clock without general anaesthesia, without lengthy recovery, and without the risks of a surgical facelift. But they work in entirely different ways, produce results on very different timelines, and are better suited to different patients. Choosing the right one — or understanding whether a combination approach makes more sense — starts with knowing exactly what each treatment does.
This article has been written and reviewed by Dr Shirin Lakhani MBBS MRCGP DRCOG (GMC: 4634863), founder and medical director of Elite Aesthetics and specialist in intimate health, regenerative medicine, and aesthetic treatments.
What Is HIFU and How Does It Work?
HIFU (High-Intensity Focused Ultrasound) is a completely non-invasive skin tightening treatment that uses focused ultrasound energy to stimulate collagen and elastin deep within the skin’s layers — without breaking the surface. The technology targets the SMAS layer (superficial musculoaponeurotic system), the same structural layer addressed in a surgical facelift, but does so entirely from the outside.
During a HIFU session, a handheld device is passed over the treatment area, delivering precise pulses of ultrasound energy at controlled depths — typically 1.5mm, 3mm, and 4.5mm beneath the skin. Each pulse creates a focal point of thermal energy that triggers a mild controlled injury response, prompting the body to produce new collagen and elastin fibres. The skin surface itself is unaffected throughout.
Because the results depend on the body’s own collagen remodelling process, they are gradual. Most patients begin to notice tightening and lifting at around six to eight weeks post-treatment, with results continuing to develop for up to three months. The improvements — including firmer skin, reduced jowling, a more defined jawline, and smoother neck — can last between 12 and 18 months from a single session, with some patients achieving results closer to two years.
HIFU is FDA-cleared for facial lifting and is one of the most extensively studied non-surgical aesthetic treatments available. A 2025 systematic review published in the Aesthetic Surgery Journal — analysing 45 clinical trials — confirmed improvements in skin laxity of 18 to 30% in the lower face, neck, and periorbital regions, measured at the six-month peak of collagen remodelling. In one large study, 85.3% of participants reported overall satisfaction with HIFU results.
HIFU is particularly well suited to patients with mild to moderate skin laxity who are beginning to notice jowls, a softening jawline, or general skin looseness in the cheeks, brows, or neck. It does not require any anaesthesia, though some practitioners — including our team at Elite Aesthetics — apply a topical numbing cream to maximise comfort. There is no downtime; most patients return to work and normal activities the same day.

What Is a Thread Lift and How Does It Work?
A thread lift is a minimally invasive procedure in which fine, dissolvable sutures — most commonly made from polydioxanone (PDO) — are inserted beneath the skin using a thin needle or cannula. These barbed or cone-shaped threads are then gently repositioned to physically lift and reposition sagging tissue, producing an immediate mechanical lifting effect visible from the moment of treatment.
Unlike HIFU, the lifting action of a thread lift is structural and immediate. The threads act like an internal scaffolding, anchoring and repositioning the soft tissue. As the threads gradually dissolve over four to six months, they leave behind a collagen-rich matrix that helps maintain the lift and improves skin quality over time. The dual action — immediate lift plus longer-term collagen stimulation — is one of the treatment’s key advantages.
A thread lift procedure typically takes 45 to 90 minutes and is performed under local anaesthesia. There may be mild bruising, swelling, or tenderness for a few days after treatment, making the recovery period slightly longer than HIFU. Most patients can return to work within two to four days. Full results are visible at six to twelve weeks, and with proper aftercare, the lifting effect typically lasts between 12 and 18 months. Maintenance treatments can be carried out after approximately one year.
Thread lifts are particularly effective for patients with moderate skin laxity who want to see a meaningful, structural lift — particularly in the mid-face, jowls, brows, or neck — that would be difficult to achieve with energy-based devices alone. They can also be used to address very specific areas such as the eyebrow, corners of the mouth, or nasolabial folds.
Our full range of thread lift options is available on our thread lift for face and neck page, including PDO threads for different areas of the face.

Who Is the Ideal Candidate for Each Treatment?
The right treatment depends on several patient-specific factors, including the degree of skin laxity, treatment goals, tolerance for downtime, and whether an immediate or gradual result is preferred. The following criteria help clarify which treatment tends to be the better fit.
HIFU is typically the better choice if you:
- Have mild to moderate skin laxity rather than significant drooping or sagging
- Want a completely non-invasive procedure with zero downtime and no needles
- Are happy to wait 6–12 weeks for gradual, natural-looking results to develop
- Want to address overall skin quality — including fine lines, texture, and tone — as well as lift
- Are in your 30s, 40s or early 50s and want a preventative or early-intervention approach
- Have previously had a thread lift and want to extend and maintain the results
A thread lift is typically the better choice if you:
- Have moderate to significant skin laxity — particularly in the jowls, mid-face, or brows — that requires structural repositioning
- Want to see a visible lifting result immediately after treatment
- Are willing to accept 2–4 days of mild downtime for a more pronounced lift
- Are targeting a specific area — such as the brow, the corners of the mouth, or the jawline — rather than overall tightening
- Have had HIFU previously and want to build on the collagen foundation with a structural lift
It is worth noting that neither treatment is appropriate as a substitute for surgical intervention when laxity is severe. Patients with very significant skin excess or deep jowling may benefit more from a consultation with a surgeon. Our medical team will always provide an honest assessment of whether a non-surgical approach can realistically achieve your goals.
What Results Can You Expect — and How Long Do They Last?
Results from both treatments are real, documented, and backed by clinical evidence — but they look and feel different from one another, and the timeline matters.
With HIFU, the experience in the first few weeks post-treatment can be underwhelming for patients who are used to seeing instant results. The treatment has effectively set a collagen-remodelling process in motion, but the skin surface looks the same immediately afterwards. Most patients begin to notice firming and tightening from around six weeks, with the full effect visible at three months. After that, results continue to hold for 12 to 18 months and, in some patients, approaching two years. Our HIFU before and after results blog covers what the typical patient journey looks like at each stage.
With a thread lift, the lifting effect is immediate. Patients often leave the clinic able to see the difference in the mirror on the same day. There may be some temporary swelling that mildly distorts the result in the first week, but this resolves and the final result — incorporating the early collagen response — is visible at around six to twelve weeks. Thread lift results also last 12 to 18 months, with annual maintenance procedures recommended to sustain the effect.
A key clinical development in 2025 has reinforced the value of combining both treatments. A retrospective observational study published in Plastic and Reconstructive Surgery – Global Open (June 2025) evaluated 58 participants who received a combination of thread lifting and HIFU across three treatment sessions. Using validated Merz aesthetic scales, the study demonstrated statistically significant improvements in physician-evaluated facial appearance — and found that the combination approach prolonged the overall treatment effect compared with either treatment in isolation. This was the first study specifically designed to evaluate the combined use of both modalities for facial aging.
For patients who want the best of both — the immediate visible lift of a thread lift and the deep collagen remodelling of HIFU — a staged or combined approach is now supported by emerging evidence and is a treatment pathway we offer at our clinic in Greenhithe, Dartford, Kent — 22 minutes from London.
Can You Have HIFU and a Thread Lift Together?
Combining HIFU and a thread lift is not only possible — it may produce better and longer-lasting results than either treatment alone. The two modalities are complementary rather than competing: HIFU works at the level of collagen and elastin synthesis to improve skin quality and tone, while threads physically reposition and anchor the lifted tissue. Together, they address both the structural and the biological aspects of facial aging.
The sequencing of combined treatment matters. Some practitioners perform HIFU first to prime the skin’s collagen environment, then follow with threads once the skin has begun to remodel. Others prefer to place the threads first and add HIFU at a later date to extend the results. The 2025 PMC study referenced above found that performing both on the same day did not negatively affect outcomes in the three-session protocol studied.
At Elite Aesthetics in Greenhithe, Dartford, Kent — 22 minutes from London — Dr Shirin Lakhani provides a thorough consultation before any combination treatment to determine the right sequencing for each patient. Complementary treatments such as Profhilo skin booster can also be incorporated to further improve skin hydration and elasticity alongside a lifting programme.

Frequently Asked Questions About HIFU vs Thread Lift
Neither treatment is universally better — the right choice depends on your degree of skin laxity, how quickly you want to see results, and your tolerance for any downtime. HIFU is better for patients with mild to moderate laxity who want a completely non-invasive procedure and are happy for results to develop gradually over 6–12 weeks. A thread lift is better for patients who need a more structural, immediate lift — particularly in the jowls, brows, or mid-face — and are prepared for a few days of mild recovery. For some patients, combining both treatments produces the best outcome. To find out which approach is right for you, call us on 01322 381 205 | WhatsApp Us | info@elite-aesthetics.co.uk
Both HIFU and thread lifts typically last 12–18 months, though individual results vary. HIFU results develop gradually — peaking at around 3 months — and can last up to 2 years in some patients. Thread lift results are visible immediately and also last 12–18 months, with annual maintenance procedures recommended. Combining both treatments has been shown in a 2025 clinical study to prolong the overall lifting effect compared with either treatment alone.
For established jowls with moderate skin laxity, a thread lift often produces a more immediate and structurally pronounced improvement because it physically repositions and anchors the descended tissue. HIFU is effective for early or mild jowling and works well as a preventative measure or to maintain and extend thread lift results. Patients with significant jowling may benefit most from a combination approach — or may require a surgical assessment if the degree of laxity is beyond what non-surgical treatments can realistically address.
A thread lift involves local anaesthetic injections which eliminate discomfort during the procedure itself, though some patients experience tenderness and bruising for 2–4 days afterwards. HIFU requires no injections — a topical numbing cream is applied beforehand — and patients typically describe the sensation as a deep, warm pressure or brief stinging at the ultrasound focal points. Most patients find HIFU more comfortable during the procedure, while a thread lift may have a slightly longer post-treatment recovery period.
Yes — combining HIFU and a thread lift is both safe and clinically supported. A retrospective study published in Plastic and Reconstructive Surgery – Global Open (June 2025) found that patients receiving a combination of thread lifting and HIFU across three sessions showed statistically significant improvements in physician-evaluated facial appearance, and that the combined approach prolonged the overall treatment effect compared with either treatment in isolation. At Elite Aesthetics in Greenhithe, Kent, Dr Shirin Lakhani can advise on the right sequencing for your individual anatomy and goals.
Most patients achieve excellent results from a single HIFU session, with the effects lasting 12–18 months. Patients with more significant skin laxity may benefit from 2–3 sessions, spaced several weeks apart, to achieve the desired degree of tightening. Annual maintenance sessions are commonly recommended once the initial results begin to gradually diminish. At Elite Aesthetics, Dr Lakhani will assess your skin and recommend the appropriate number of sessions at your consultation.
Thread lift results are visible immediately after the procedure. There may be mild swelling or bruising in the first few days that temporarily affects the appearance, but once this resolves — typically within one to two weeks — the lift is clearly visible. Results continue to improve over the following 6–12 weeks as collagen forms around the thread sites, enhancing skin quality alongside the structural lift.
Both HIFU and thread lifts have well-established safety profiles when performed by appropriately trained medical practitioners. HIFU is FDA-cleared for facial lifting and has been evaluated in over 45 clinical trials. Thread lifts use fully dissolvable PDO sutures that are absorbed safely by the body over 4–6 months. As with any aesthetic procedure, risks are minimised significantly when treatment is carried out by a qualified doctor rather than a beauty therapist. At Elite Aesthetics, all procedures are performed or directly supervised by Dr Shirin Lakhani MBBS MRCGP DRCOG (GMC: 4634863) in a CQC-registered clinic.
